PARTICIPANT RELEASE INFORMATION
EACH PERSON PARTICIPATING IN THE SHOOT OUT MUST SIGN A PARTICIPANT RELEASE FORM.
Due to the strict procedures necessary to bring this special event to Boulder, each participant must sign and turn in the Participant Release (found on the last page of the Entry Form).
Clearly record each participant’s name, address, contact information, and category (Producer, Director, Scriptwriter, etc.) on the entry form. The Shoot Out Boulder awards prizes for each category, and you must be a registered team member to be eligible to win. Any participant whose entry information is incomplete WILL NOT BE REGISTERED and therefore not considered part of the event.
Team members can be listed for more than one category, e.g., one person can be your team’s Producer, Cameraperson, and Art Director. You can even be a one-person team, listing your name for all categories. There is no additional fee for this, but there must be A NAME ENTERED FOR EACH CATEGORY—even if it means duplicating information.
Participants can work with more than one team but must register as part of each team.

The entry fee covers one participant. Additional team members pay a per-person rate (see fee structure below). Each registered team member receives (1) one comp ticket for the screening of the top ten films Sunday's screening at the Boulder Theater. You may purchase additional tickets for $14.75 each.
There are a couple different fee structures for entry. Please review before filling out the application in order to see if you qualify.
ENTRY FEES
$50.00 for 17 and UNDER. Each additional team member is $20.00 (two-thirds
of the team must be 17 and under to qualify).
$100.00 for 18 and OVER. Each additional team member is $25.00.
$125.00 for 3 members OF THE SAME FAMILY. Each additional team member is
$20.00.
ALL REGISTERED TEAMS WILL RECEIVE A GRAB BAG AT THE START OF THE EVENT. One
bag per team.
Proof of age, student I. D., and/or Family relationship may be requested.
One scenario: Paul Smith and Sue Jones are a film-making team. They list Jones as Director and Smith as responsible for all other categories. They will pay $100 for the first person and $25 for each additional team member. So the entry fee for their two-person team is $125. If Jones and Smith enter as a “17 and under team,” the fee will be $50 for the first person and $20 for each additional team member—for a total of $70.
**A City of Boulder admission tax fee of 5% is being collected for all individual and team entries. This amount is added to the final amount due.
Payments must be mailed by Wednesday, October 8, or paid online by midnight Thursday, October 9. After that time entry forms and payment must be delivered in person on Friday, October 12 between 4:00 and 6:00 pm at the sign-in booth, located at near the courthouse on the Pearl Street Mall.
WHAT IS THE SIGN-IN/TAPE RETURN PROCESS?
For sign-up, all that is required is a registration of the team along with payment which is generally done by the named producer.
Additional team members can be added at sign-in on Friday [4-6 PM] and then again at tape return on [4 PM - 6 PM] Saturday. You will return the tape to the same place that you picked it up. The information is organized by the producer's last name as opposed to title of the film.
Only the producer is required to be at sign-in and tape return to insure that all documents are correct and provide an official signature.
It is suggested that the producer also collect the necessary paperwork from the team members such as the participant releases prior to sign-in.
If you are entering any time after midnight on Thursday, October 9, bring the form to the registration kiosk starting 4 PM until 6 PM, on Friday, in front of the Boulder Court House on the Pearl Street Mall in Boulder.
Use your credit card via MASTERCARD/VISA PayPal to make your online payment. If you do, use the online registration form. If you plan on mailing a check, make sure you do it by Wednesday, October 8, 2008.

This is a rain or shine event. There are no refunds or rain checks. If there is a delay in starting due to festival issues, the end of the event will be 24 hours later i.e. 6:15 PM Friday until 6:15 PM Saturday. There are also no refunds if you are late, it is a contest.
Please note:
It is recommended that all participating actors are registered, but The Shoot
Out Boulder also realizes that the spontaneity of some of the decisions made
during the competition makes this recommendation difficult to implement.
Therefore The Shoot Out makes it the Producer’s responsibility to make sure all ‘extra’ cast members sign a ‘Accident Waiver and Release of Liability’ disclaimer. The Shoot Out can provide a simple disclaimer form available upon request. If you are considering a large cast, it may be worthwhile including this procedure as part of your pre production.
ANY ACTORS WHO WISH TO BE CONSIDERED FOR AWARDS WILL NEED TO BE OFFICIALLY REGISTERED FOR THE EVENT. YOU CAN HAVE TEAM MEMBERS THAT ARE NO REGISTERED BUT THEY WILL NOT RECEIVE A COMP TICKET TO THE SUNDAY NIGHT EVENT NOR WILL THEY BE ELIGABLE FOR ANY OF THE CATAGORIES.
Actors have two different ways to be involved; one, as a member of a registered team, you are eligible in the Best Actor Category.

The Shoot Out Boulder does not provide any protection or insurance coverage against any type of personal injury, damage to property or any other liability, third party or otherwise.
LOCATION AND TALENT RELEASE FORMS
The following two forms may be used for Talent Release and Location Release. These forms are provided only as a courtesy examples of standard industry forms.
The Shoot Out Boulder awards prizes for each nominated category on the entry form. The Shoot Out Boulder's Top Ten Films will be screened in Australia and New Zealand as part of the on-going International Shoot Out 24 Hour Filmmaking Festival.
All registered participants are eligible for the awards. If a participating team is not properly registered that participating team is not eligible for the award.
